Open Meadow Days & Property Visit Liability Waiver and Release
This Waiver and Release of Liability is executed by the undersigned parent or legal guardian on behalf of themselves and any minor child(ren) listed below, in favor of Hidden Meadow School, its owners, staff, volunteers, and representatives.
1. Acknowledgment of Risk
Participation involves inherent risks including uneven terrain, mud, weather, water, open fires, and physical activity. Participation is voluntary.
2. Assumption of Responsibility
I am responsible for supervision of my child(ren) and any guests at all times. No childcare or supervision is provided.
3. Release of Liability
I release and hold harmless Hidden Meadow School from any claims arising from participation, to the fullest extent permitted by law.
4. Medical Authorization
I authorize emergency medical care if needed and accept responsibility for all costs.
5. Water Activities
Swimming is not permitted. Watercraft use requires proper life preservers and is at participant’s own risk.
